First, call and speak with the counselor to schedule an appointment
Then complete the forms linked below on this page
This free download of Adobe Acrobat Reader can assist in completing and signing your documentation electronically
Email completed forms at least 24 hours prior to your first appointment.
Gmail offers an encrypted email option if you are uncomfortable sending as a normal attachment.
Therapy Portal
After your paperwork is processed, you will receive an email from Therapy Portal.
Create a profile (this is necessary in order to attend your telehealth session).
Complete the Payment Authorization Form in your Therapy Portal prior to your appointment
Attending Your First Session

Click “Join Session”
Returning Clients
Consecutive appointments

Click “Request Appointment”
Choose an available time
requesting an appointment blocks that time on the calendar so that no one else can schedule.
Pause in Counseling
Portal accounts archive after 60 days without a session.
Please contact the counselor if you wish to continue after a pause in counseling.
